How can I take part in this? 1. You must be receiving the l/0 or Level One Waiver, or be willing to enter into ICFMR or private pay agreement. 2. Contact Amanda Kennedy at 614.291.9411 ext.516 or 614.439.7820 or email firstname.lastname@example.org to schedule an enrollment meeting.
How will I get there? All activities will occur on the Ohio State University campus. Accessible transportation will be provided to and from your front doorstep if needed.
What are the hours? The hours and days are varied, and are based solely on the choices of the persons served. Most activities occur in the afternoons and evenings, Monday to Friday, and various times on Saturday. Activities can be regularly scheduled, or one-time events.
What are the fees? Payment options 1-l/O or Level 1 waiver: Services and Transportation will be billed to the waiver. While there is no fee for most activities, there is a $5-10 out of pocket cost to you per 3 hour RPAC outreach session. The RPAC is the Recreational and Physical Activities Center, and the fee covers professional instruction. The RPAC outreach fee must be paid by participants at the time of arrival for each activity.
Payment option 2: $12 per hour will be billed to families monthly for ADD staff services, and $39.40 per round trip will be billed monthly for transportation if used. While there is no fee for most activities, there is a $5-10 out of pocket cost to you per 3 hour RPAC outreach session. BUCKEYE CONNECTIONS is the lastest resource for persons with developmental disabilities in the Central Ohio area. ADD has teamed up with the Ohio State University to bring the Buckeye experience to adults with developmental disabilities.
